Email Request Form

A simple and yet a professional looking email request form written in php script. It asks for sender's name, email, category, type, subject and message. It does preview, validate form fields, send autoresponder to sender and a copy to administrator. Sender's IP address and browser information are shown on the admin letter. Personalize your emails by adding your website name, url, and company message. As an added bonus, we've included a feedback section in case your users would like to let you know what they think of your site. Great script for PHP newbie who wants to learn how to work with php form processing scripts.
