Uses templates for customizing to match the look of your site. Also makes upgrading easier. Allows you to choose if the entries should be displayed on the form page or on a page of it's own. Allows you to choose the layout of the displayed entries. If a person entering does not wish to provide a name, their listing is displayed as anonymous. Email is NOT displayed to prevent spammers from attaining your entrants email addresses. Email is stored in the database for your own information. Currently Being developed: Ability to block abusive users by their email or ip address. Requirements: PHP and MySql
